in the field each week. The note will be in the S.O.A.P. format.
Normally section S contains subjective information provided by the
client system; O is the objective information of the treatment; A is the
clinician’s analysis or synthesis of the S and O sections; and P is the
clinician’s plan for the next session with the client system. For a
